BENEFIT PLANS

JULY 1, 2018 – June 30, 2019

The Archdiocese of Baltimore (AOB) has designed its benefits programs to provide our employees and their families with a comprehensive health and welfare package, including medical; dental; vision; life and accidental death and dismemberment; short-term and long-term disability; employee assistance program; and retirement plans. Eligibility for the plans generally depend on an employee’s benefit eligible status of at least 20 hours per week for 12-month employees or at least 25 hours per week for 10-month employees.

For the 2018-2019 Plan Year, the Archdiocese offers the following benefit plans. For more details on each plan, please scroll down to the 2019 Benefit Documents.

2018-2019 Benefit Plans

Medical Plans

(include prescription coverage through CVS Caremark and vision coverage through VSP)

 

CIGNA Open Access Plus Plan

 

 

This plan utilizes the CIGNA OAP network of providers.  Employees may choose to go to any provider, but will incur higher costs if using an out-of-network provider.  Certain tests and procedures will require prior authorization.

 

CIGNA High Deductible Health Plan (HDHP) with Health Savings Account (HSA)

This plan also utilizes the CIGNA OAP network of providers, but has a higher deductible and a lower premium than the OAP Plus Plan. Once employees meet the high deductible, the plan begins paying medical and prescription claims.  Employees may elect to make pre-tax contributions to their HSA to use for qualified medical expenses.  The Archdiocese will provide a discretionary quarterly employer contribution to the HSA that is subject to change from year to year.

Dental Plans CIGNA Dental PPO

This plan utilizes the CIGNA PPO dental network of providers.  Employees may choose to go to any provider, but will incur higher costs if using an out-of-network provider.

 

United Concordia Dental HMO

This is an HMO plan and employees will need to select a participating primary care dentist for each family member.  That primary dentist will refer all specialist care.  If employees do not designate a primary dentist at the time of enrollment, UCCI will designate one for all family members, which employees may change at any time.

Vision Plans

 

VSP Vision included in Medical

If employees enroll in medical, vision is automatically included.  Employees do not need to make a separate election unless enrolling different family members who are not enrolled in the medical plan.

VSP Standalone Vision

If employees and/or family members do not enroll in the medical plan, but would like vision coverage, employees may enroll in this standalone plan.

Disability Plans

 

CIGNA Short-Term Disability

Employees are eligible for this employer paid benefit after six months of service. Coverage is up to 26 weeks following date of disability.  A waiting period applies and is dependent on accrued sick time.

CIGNA Long-Term Disability

 

If the disability continues past 26 weeks, employees may be eligible to continue long-term disability.

Employees are guaranteed issue of supplemental and spousal life insurance upon hire or benefit eligible status; if enrollment occurs later, employees are subject to medical underwriting

Supplemental Life – Employee paid voluntary employee life insurance at .5, 1x, or 2x employee’s benefit salary.

Spousal/Dependent Life – Employee paid voluntary dependent life insurance.

Retirement PlansArchdiocese of Baltimore and Catholic Charities employees who were participants in the frozen Lay Employee Pension Plan as of June 30, 2011 may also be eligible for a pension benefit.  Please contact the Benefits Office at insure@archbalt.org with any questions.

Catholic Charities employees have a separate 403(b) plan and may contact their local Human Resources department for information.

T. Rowe Price 403(b) Employee Contribution Plan

 

Archdiocese of Baltimore benefit eligible and non-benefit eligible employees may elect to contribute to the 403(b) plan on a pre-tax basis.  There is no waiting period.

Archdiocese of Baltimore benefit eligible employees who are at least age 21 are eligible to participate effective July 1 or January 1, whichever immediately follows the date of hire or change in employment status.  Contributions begin on a quarterly basis following the end of the quarter in which an employee becomes a participant.

 

2019 Benefit Documents

2019 Open Enrollment Guide — Provides summaries of the medical, dental, vision, and employer provided plans; health management programs offered by Cigna; and decision support tools to assist employees in selecting the best plan for themselves and their family members.

2019 Enrollment Form – AOB employees must complete and return this form to their direct employer within 30 days of their date of hire or change in employment status.  Central Services employees should submit their forms to the Employee Benefits Office. Catholic Charities employees enroll online through UltiPro.  AOB employees enrolling in the High Deductible Health Plan who want to contribute to the Health Savings Account must contact their direct employer for the HSA Deduction Authorization form.

Life Insurance and AD&D, Supplemental Life Insurance Forms — Benefit eligible employees are automatically enrolled in employer paid basic life insurance and AD&D at time of hire or change to benefit eligible employment status.  Employees must complete the beneficiary designation form below and return it to their direct employer.  Employees may also apply for employee paid supplemental and/or dependent life insurance by completing the application below and returning it to their direct employer.  If employees enroll in supplemental life insurance when first eligible, they are guaranteed coverage.  If employees enroll themselves or spouses later, Cigna requires enrollees to answer the medical questions found in the application.  Enrollment may or may not be approved. Coverage will not become effective until it has been medically approved by CIGNA.  Children are guaranteed coverage at any time.

Disability — Benefit eligible employees are automatically enrolled in disability and are eligible for this benefit after six months of continuous employment.  If you have an accident or illness, please call Cigna at 800.362.4462 to initiate the claim.

 

Insurance Cards

Upon enrollment, employees will receive new benefit cards in the mail.  If employees need a card in the meantime, please follow the instructions as follows:

CIGNA Medical ID card – If employees need to print a temporary medical ID card after their benefit effective date (first of month following date of hire or change in status), go to www.mycigna.com, log into account and scroll down to the “Print ID card” link.  If the employee has never accessed their account before, register first and then log in with the created user name and password.

Caremark Prescription ID card – For a temporary Caremark prescription card, go to www.caremark.com, employees may log into their account, and scroll down to “Print Member ID Card” under Plans & Benefits at the bottom of the page.  If the employee has never accessed their account before, register first and then log in with the created user name and password.

United Concordia Dental ID card – If employees need a card prior to receiving it in the mail, please contact United Concordia directly at the number found in the Vendor list below.

Cards not issued – Employees will not receive the following cards in the mail, but if needed, may print a dental, vision or EAP card by clicking below.
