Prescription Drug

You automatically receive prescription drug coverage if you participate in the CIGNA OAP or CIGNA PPO Plan. Prescription drug coverage is provided through Caremark. Prescription drug coverage is only available if you are enrolled in a CIGNA medical plan with the Archdiocese. The prescription drug program offers prescription drugs at three payment levels; this program is called a “three-tiered” prescription drug plan.  The actual payment depends on whether you fill your prescription at a retail pharmacy or order it through the mail, and on whether the drug is generic, preferred brand, or non-preferred brand.

Enroll:

To enroll for medical benefits, complete the Medical/Dental enrollment form and give your completed form to your direct employer.

Important Note:

If you take a long-term, maintenance medication, you should use the pharmacy mail order service, Caremark Mail Service Program. You must use the mail order program to refill a prescription after receiving two refills at a retail pharmacy. When you use mail order, you receive a 90-day supply of your medication. To fill prescriptions through Caremark Mail Service Program, send your prescription along with the appropriate Caremark order form (available on the Caremark website) to Caremark, P.O. Box 94467, Palatine, IL 60094-4467.
