In order to be in compliance with "A Statement of Policy for the Protection of Children & Youth", the following documents should be placed in the personnel file of the employee or volunteer:

- Completed reference check documentation
- Employee Acknowledgement of Receipt and Review of A Statement of Policy for the Protection of Children & Youth and the Code of Conduct for Church Personnel in the Archdiocese of Baltimore
- Results of CJIS/Fingerprint Check
- Policy Compliance Verification for Employees form
Volunteer who has substantial contact with children file*
- Application
- Complete reference check documentation
- Volunteer Criminal History Consent form
- Driving Record Consent form, if necessary
- Policy Compliance Verification for Volunteers form
The files are to be kept in a confidential, safe and secure location with controlled access, such as in a locked file cabinet. Files may be reviewed by the appropriate supervisory personnel, but may not be copied or removed from the offices where they are maintained.
*Note: Other documentation not directly related to this policy may also be required for files.
